Messages From Your Tongue

A healthy tongue usually appears pink with a light coating. Small bumps, called papillae, give it a slightly rough surface. A change in the color, texture, and coating of your tongue may be sending you signals that something is wrong. Here are a few changes that you some patients experience.

A white-coated tongue indicates a buildup of either debris, dead cells, or bacteria. This is most commonly caused by dry mouth, smoking, or poor brushing of the teeth and tongue. To minimize buildup, regularly brush your tongue whenever you brush your teeth and stay hydrated. Seek care if you develop a heavy coating on your tongue, along with soreness under brushed areas.

A bright red or smooth tongue points to irritation or low nutrients like B vitamins or iron. If the surface of your tongue looks glossy or “bald” and feels tender, you need to improve your diet and monitor changes. Seek care if soreness or color change lasts more than two weeks.

A cracked or grooved tongue, called a fissured tongue, often comes from natural variation or aging. Food and bacteria collect in the grooves, causing odor or irritation. Clean the tongue gently each day. Seek care if pain, swelling, or persistent buildup develops.

A “hairy” tongue is typically related to the accumulation of keratin, either through excessive cigarette use, coffee consumption, as a medication side effect, or chronic dry-mouth conditions. Remove the coating using a brush or scraping tool, improve your oral hygiene, and seek care if the coating continues to persist or creates discomfort.

Sores or patches on the tongue most often occur from injury or irritation from certain foods and drinks. Most sore or patchy areas heal themselves within one to two weeks. Keep the area clean and avoid irritants. Seek care if sores last longer, grow, bleed, or show red or white patches.

Caring for Your Tongue

Taking care of your tongue requires minimal time and effort. The following are a few daily tasks recommended to greatly enhance the health of your tongue.

  • Brush your tongue daily. As part of your normal tooth brushing routine, brush your tongue as well. Most toothbrushes have a textured back for cleaning the top surface of your tongue. For more effective cleaning, use a tongue scraper available at most drugstores, pharmacies, and supermarkets with a dental health aisle.
  • Stay hydrated. A dry mouth encourages the growth of harmful oral bacteria. Drinking water throughout the day helps keep your oral environment balanced.
