1) Eligibility & Geographic Scope

• Scheduling: You may request appointments through the Site or by phone at (916) 898-0003. Appointment requests are subject to confirmation and availability.
• Changes/Cancellations: If you need to modify or cancel an appointment, please notify us as early as possible. Specific office policies (e.g., late cancellation fees) may apply and will be provided by staff at booking.
• Communications: By providing contact information, you authorize us to contact you about scheduling and related matters. Message/data rates may apply. You may request changes to your communication preferences at any time via our Contact page.

4) Insurance, Financial Responsibility & Estimates

• Coverage: Insurance information on the Site is general. Plan participation and coverage vary by carrier and plan. Verification of benefits is not a guarantee of payment.
• Estimates: Treatment estimates are provided as a courtesy and are not a guarantee of final charges or insurer reimbursement.
• Responsibility: You are responsible for all charges not covered by insurance, including deductibles, co-pays, co-insurance, and non-covered services.

5) Acceptable Use

10) No Online Payments

The Site does not offer online payment processing. If a payment is required for services, our staff will provide available options directly. Do not submit payment information through Site forms.

Promotions, offers, or informational content on the Site are subject to change without notice, may be time-limited, and may be subject to eligibility requirements or exclusions. Results and outcomes vary by patient.

16) Governing Law & Venue

These Terms are governed by the laws of the State of California, without regard to conflict-of-law principles. You agree to submit to the personal jurisdiction and venue of the state and federal courts located in Placer County, California for any dispute arising out of or relating to these Terms or the Site.

17) Miscellaneous

• Severability: If any provision is held invalid, the remaining provisions remain in full force.
• No Waiver: Failure to enforce a provision is not a waiver of the right to do so later.
• Assignment: You may not assign or transfer any rights under these Terms without our prior written consent.
• Entire Agreement: These Terms constitute the entire agreement between you and us regarding the Site and supersede prior or contemporaneous communications on this subject.
